Overview

What is revision rhinoplasty?

Revision rhinoplasty in İstanbul — also known as secondary rhinoplasty — is performed when a person has already had nose surgery but still has aesthetic or functional concerns. These can appear shortly after the first surgery or become more noticeable as the nose heals and settles.

Some people seek it because the nose still looks too large, crooked, over-reduced, pinched, asymmetrical or unnatural; others have breathing problems, nasal collapse, tip weakness or deformities from loss of support. Aesthetic and functional concerns often exist together, so revision evaluates both the outer shape and the internal structure.

What it addresses

Common reasons for revision rhinoplasty.

People consider revision for different reasons — dissatisfaction with the shape after the first surgery, or breathing difficulty and structural weakness. The goal isn’t always a dramatic change; often it’s a more natural, stable nose that suits the face. Common concerns:

  • A crooked nose
  • An irregular nasal bridge
  • An over-reduced nose
  • A drooping nasal tip
  • A pinched tip
  • Nostril asymmetry
  • Visible deformities
  • Nasal collapse
  • Persistent breathing problems

Why it’s different

More complex than a primary rhinoplasty.

Primary rhinoplasty is performed on a nose that hasn’t been surgically changed. Revision is different — the anatomy is no longer untouched. The previous surgery may have removed cartilage, changed the bridge, weakened the tip support or created internal scar tissue.

Scar tissue can make tissues less flexible, the skin may not adapt as easily, and cartilage support may be limited — especially if too much was removed before. This is why revision requires more detailed planning: understanding what was done, what the current problem is, and what can be safely corrected.

  • Altered anatomy

The previous surgery may have removed cartilage, changed the bridge or weakened the tip support.

  • Scar tissue

Less flexible tissues and skin that doesn’t adapt as easily as before the first surgery.

  • Detailed planning

Understanding what was done, what the problem is, and what can be safely corrected.

Experience

Why experience matters in revision.

Revision rhinoplasty is one of the areas where surgical experience has a direct impact on planning. The surgeon must understand not only the current appearance of the nose, but also the changes created by the previous surgery.

Across nearly 30 years of surgical experience, Dr. Nuran Kalekoğlu ENT Specialist | Rhinoplasty & Facial Aesthetic Surgeon has evaluated and operated on many primary and revision cases of differing difficulty — focusing not only on the external appearance, but on breathing function, skin structure, existing cartilage support and the structural changes left by the previous operation.

Success depends on correct case selection, experienced planning, individual analysis and a patient healing process.
Personalised planning

A plan built around your nose.

Every revision nose is unique, so the evaluation before surgery is extremely important. Dr. Nuran Kalekoğlu ENT Specialist | Rhinoplasty & Facial Aesthetic Surgeon examines the nose in detail, listens carefully to your expectations, and plans realistically — based on what is medically possible, structurally safe and aesthetically suitable for the face.

Assessed in your plan

  • The nasal bridge
  • The nasal tip
  • Nostril shape
  • Skin thickness
  • Breathing function
  • Facial proportions
  • Existing cartilage support
  • The previous surgical changes
The technique

The open technique — for precise reconstruction.

Dr. Nuran Kalekoğlu ENT Specialist | Rhinoplasty & Facial Aesthetic Surgeon performs rhinoplasty with the open technique. In revision rhinoplasty it gives detailed visibility of the nasal structures and helps with precise reconstruction.

  • Detailed visibility

The previous changes in cartilage and bone are evaluated directly, under controlled access.

  • Precise reconstruction

An irregular bridge refined, a weak tip strengthened, a collapsed or narrowed nose rebuilt.

  • Built to last

Structural support is restored where needed, for a stable as well as natural result.

The procedure

How is revision rhinoplasty performed?

Revision is performed according to the needs of the individual nose. Because it has already been operated on, the plan may involve reshaping existing structures, rebuilding weakened support or correcting irregularities. During surgery the previous changes in cartilage and bone are evaluated — an irregular bridge refined, a weak tip strengthened, a collapsed or narrowed nose rebuilt — and when the remaining cartilage isn’t enough, additional support may be taken from the ear or rib.

Reshape & rebuild

Existing structures reshaped, weakened support rebuilt, irregularities corrected.

Grafts when needed

Extra cartilage from the ear or rib when the remaining support isn’t enough.

Breathing supported

Internal structure evaluated and rebuilt to protect or improve airflow.

Cartilage grafts

Rebuilding support when the nose needs it.

Cartilage grafts are often important in revision, because the nose may need additional support — in the first surgery some cartilage may have been removed or weakened. Grafting doesn’t only improve appearance; it rebuilds strength, improves stability and supports breathing.

Septal cartilage

Used first when enough remains inside the nose.

Ear cartilage

Useful for selected contour and support needs.

Rib cartilage

Preferred when stronger structural support is required.

Form & function

A better nose should breathe better too.

Breathing problems are one of the most important concerns in revision. A previous surgery may have changed the internal structure, weakened support or narrowed the airway.

So nasal breathing is evaluated together with the external appearance — septum deviation, internal valve weakness, collapse or obstruction addressed as part of the plan. A nose that looks better but doesn’t function well isn’t a complete result.

Aesthetic role

A natural nose that fits the face after a previous operation.

Functional role

Airflow, septum and internal support evaluated and rebuilt where needed.

Recovery

How long is recovery after revision rhinoplasty?

Recovery after revision is usually more gradual than primary — because the tissues have been operated on before, swelling may last longer and the nose may take more time to settle. A splint supports the nose at first, and what you see when it’s removed is not the final shape.

Realistic expectations

Honest planning, within the existing anatomy.

Revision can improve many aesthetic and functional problems, but it should be planned with realistic expectations. A previously operated nose has limits, and these are explained clearly before surgery.

The aim isn’t always a perfect nose — it’s the best possible result within the existing anatomy, tissue condition and structural needs. Your concerns are understood carefully and the outcome planned honestly.

A successful revision depends on trust, patience and a clear understanding of what can be achieved.
Already had surgery?

Having had surgery before doesn’t mean there’s no solution.

Having had nose surgery before does not mean nothing can be done. If you’re unhappy with how your nose looks, or you have difficulty breathing, a detailed evaluation can show what can be improved.

Dr. Nuran Kalekoğlu ENT Specialist | Rhinoplasty & Facial Aesthetic Surgeon evaluates each revision case individually — the existing problems, previous surgical changes, breathing function and facial harmony considered together. The goal is a nose that doesn’t look operated, looks natural with the face, and supports breathing as much as possible.
